Spain has an excellent public healthcare system (Sistema Nacional de Salud), but access for non-EU residents depends on your immigration status and Social Security contributions. Understanding your options before you arrive — and choosing the right health insurance for your visa application — can save significant time and money.
Health Insurance as a Visa Requirement
Non-Lucrative Visa, Digital Nomad Visa, Student Visa, and Self-Employment Visa applicants must hold private health insurance with full coverage in Spain and no co-payments or deductibles. Transitioning to Public Healthcare
Once you are employed in Spain or registered as autónomo, you contribute to Spain's Social Security system and gain the right to access the public healthcare system. Non-Lucrative Visa holders and Golden Visa holders who do not work in Spain maintain private health insurance throughout their residency. Annual costs for comprehensive private health insurance in Spain typically range from EUR 800 to EUR 2,500 per adult, depending on age and coverage level.
